Rivaroxanban Therapy in a Patient with Left Ventricular Thrombus and Multiple Systemic Thromboembolisms Caused by Antiphospholipid Syndrome

Abstract
A 33-year-old man presented with right flank pain. Abdominal computed tomography (CT) showed renal infarction, and left ventricular thrombus was discovered by 2D-echocardiogram in our hospital. He had been taking an antiplatelet medication including aspirin for previous myocardial infarction. Despite intravenous unfractionated heparin and warfarin, recurrent thrombotic events of cerebral infarction and critical limb ischemia occurred during the next 2 weeks. More effective anticoagulation was required, and the patient was switched to rivaroxaban instead of unfractionated heparin and warfarin. This combination was helpful in preventing additional thromboembolic events. Based on clinical manifestations and the presence of lupus anticoagulant, the final diagnosis was ¡®antiphospholipid antibody syndrome¡¯ for which additional thromboembolic events were successfully managed with the use of rivaroxaban.
